Radiology Partners LLC (RPLLC) will be providing your x-ray services during your visit/ procedure. Please carefully review the following information so we may address any concerns or questions you may have.
- The charges for a Xray services are not included in the physicians/surgery fee or the facility/Surgery Center fees; we contract separately from your doctors office or Surgery Center. As a result, you may see separate charges or fees on your insurance statement for a Xray services RPLLC has provided during your procedure.
- We accept most insurances, Worker’s Compensation, and Medicare; however, we are not participating members with all insurances. Therefore, a reimbursement check may be sent to you and not to RPLLC. Please know that you are responsible for forwarding these payments to Radiology Partners LLC. ( 3160 Castro Valley Boulevard Suite 235, Castro Valley, CA 94546)
- If we are contracting as an “in network provider” with your insurance, we will except all insurance payments from them and will not collect from you for covered services more than the applicable deductible and/or coinsurance.
- Please provide us with the most recent copy of your insurance information and a copy of your insurance card.
